Applicants who interested to apply for Certified Industrial Designer shall use Form A as follows:

   Submission Requirements:

   Submit a copy of up to date Curriculum Vitae

   Identity Card (ID)

   Submission Requirements:

   A Copy of Identity Card

   Photo

   Submission Requirements:

   Photo from chest level (full colour)

   A non-refundable Fee

   Application + Evaluation Fee RM200.00
   Certification Fee RM200.00
   Yearly Renewal Fee RM200.00

   Submission Requirements:

   Application without payment will not be processed. Payment made payable to "SIRIM Berhad" can be done by transfer to Account No: 8001940224 - CIMB

   Please enclose a copy of the receipt.

   Academic Qualification

   Submission Requirements:

   A Certified Copy of Certificate stated in the application requirements.

7. 7

   Professional Credentials or Recognition
   (If any)

   Submission Requirements:

   A Certified Copy of Certificate (Only for National or International level)

   For Practicing Industrial Designers not less than 5 years experience:

   • Commercially Produced Products
   • Intellectual Property Rights (IPR)

   Submission Requirements:

   Portfolio of Evidence
   Minimum 1 Commercial Produced Products with Product Development Process, Project Management Process specifically in participating scope of practice.

   Copy of Document:
   Industrial Design / Patent / Trademark / Copyright application filed in any Intellectual Property Office in Malaysia or any other country as an applicant or author (designer) of the design (if requested)

9. 9

   For Academicians under Education Category not less than 5 years experience:

   • Commercially Produced Products
   • Research Output
   • Intellectual Property Rights (IPR)

   Submission Requirements:

   Option 1

   Portfolio of Evidence
   Minimum 1 Commercial Produced Products with Product Development Process and Project Management Process

   Portfolio of Evidence
   Minimum 1 Research Output has been applied / used by industry or government; etc.

   Evidence
   Copy of Document:
   Industrial Design / Patent / Trademark / Copyright application filed in any Intellectual Property Office in Malaysia or any other country as an applicant or author (designer) of the design (if requested)

   
   

   Option 2

   Portfolio of Evidence
   Minimum 3 Research Output has been applied / used by industry or government; etc.

   Industrial Design / Patent / Trademark / Copyright application filed in any Intellectual Property Office in Malaysia or any other country as an applicant or author (designer) of the design (if requested)

10. 10

    For Practicing Designer under Management Category not less than 5 years experience:

    • Project Output
    • Intellectual Property Rights (IPR)

    Submission Requirements:

    Portfolio of Evidence
    Minimum 5 Projects Output has been applied / recognized / used by government / industry / education such as:

    • Policy / Act / Standard / Guideline / Curriculum
    • Talent Development
    • Innovation And Entrepreneurship
    • Business For Community
    • Recognition
    • Design Development
    • Etc.

    Copy of Document:
    Industrial Design / Patent / Trademark / Copyright application filed in any Intellectual Property Office in Malaysia or any other country as an applicant or author (designer) of the design (if requested)
